Stock Information

Reregistration of Stock:  Contact Two Rivers Financial Group, 222 N. Main Street, Burlington, IA  52601 or call Donna Beck at 319-753-9151.  There is a nominal fee of $20.00 per certificate.   A Medallion signature guarantee is required on the certificate itself or a signed and guaranteed Stock Power.  A letter of instruction should be included describing the new or changed registration name, address, SSN and any other applicable information.

Lost Certificate – Please fill out the Lost Security Affidavit and Indemnity Agreement making certain it is notarized.  After completion, mail to TRFG, 222 N. Main, Burlington, IA  52601 Attn:  Donna Beck along with your check for $20.00.  The replacement certificate will be sent to you via registered mail. 

Dividend Direct Deposit – If you do not have direct deposit for dividend payments set up at this time and would like to take advantage of this efficient and timely payment method, please fill out the Automatic Deposit Authorization form making certain all numbers are accurate and legible, sign and mail to TRFG at 222 N. Main Street, Burlington, IA  52601, Attn:  Donna Beck

Annual Stockholders Meeting:  Held each January at date and time to be announced in December shareholder letter.
